This form is for students who wish to appeal a dismissal based on Satisfactory Academic Progress (SAP) for both Academic and Financial Aid purposes. If you believe your performance was affected by serious and unexpected circumstances, you may submit this appeal for review. Please complete all sections clearly and honestly, and attach all required documentation. Incomplete forms may delay or prevent review.

Explain what has changed in your situation that will now allow you to meet SAP standards moving forward.

This may include:

- Changes in schedule, support, or responsibilities
