Loan request form - Year 2

By signing below, I acknowledge and understand the following:
1. My tuition is fully covered by any applicable state and federal grants and the Maestro Pro scholarship. Any Federal Direct Loan I borrow will be disbursed to me for living or educational expenses, and will create a legal debt with interest that I must repay.
2. I will borrow only the minimum amount necessary for essential educational expenses.
3. My loan interest rate is set by federal law and may change each year for new loans.
4. Unsubsidized Loans accrue interest from the time funds are disbursed. Unpaid interest may be added to the principal (capitalized), increasing the total cost.
5. Deferment or forbearance does not stop interest from accruing and will increase the overall loan cost.
6. Federal origination fees will be deducted before I receive my loan funds, reducing the net amount disbursed.
7. Annual and lifetime borrowing limits apply. Loans I take now will reduce future eligibility.
8. I must repay my loan even if I do not complete my program, cannot find employment, or am dissatisfied with my education.
9. Failure to make payments may result in default, collections, wage garnishment, tax refund offsets, loss of aid eligibility, and damage to my credit.
10. Federal Direct Loans generally cannot be discharged in bankruptcy except in rare circumstances.
11. I must complete Entrance Counseling and sign a Master Promissory Note (MPN) before receiving loan funds.
12. I consent to receive loan disclosures and notifications electronically at the email address I provide.

Federal Direct Loan Eligibility Information

Based on current federal annual borrowing limits for eligible Year 2 students:

Maximum Direct Subsidized Loan eligibility: up to $4,500 per academic year Maximum Direct Unsubsidized Loan eligibility: up to $6,000 per academic year

Actual eligibility may be lower based on federal regulations, dependency status, cost of attendance, aggregate borrowing limits, SAP status, and remaining financial aid eligibility.
